How to Fill Out the Touro University Request for Exemption from Vaccination

**Exemptions must be submitted by the first day of classes**

      1. Log into Touro One. On the home page, click on “Covid-19 Updates & Vaccination Policy”.

      2. The Touro University Covid-19 information page will open. Scroll down until you see two blue rectangles that read “Covid Screening” and “Vaccine Verification”.  Click on “Vaccine Verification”.

      3. The vaccine verification form will open. Click “Got it”, then read the Electronic Record and Signature Disclosure message.  After review of the disclosure, check the agreement box.  Click continue.

      4. Begin to fill out the requested information on the form (name, Touro email, ID number, vaccine info). For the question, “Have you received a Covid-19 Vaccine?” click the dropdown and select the option, “No I would like to file for exemption”.  Click on, “Click here to request an exemption”.  In the “External Link Warning” window, click continue.

      5. The exemption request form will open. Click “Got it”, then read the Electronic Record and Signature Disclosure message.  After review of the disclosure, check the agreement box.  Click continue.

      6. If you are filing a medical exemption, please refer to the first four pages of the form. The first two pages must be completed by the student, the last two pages must be completed by the student’s physician.  The completed medical request must be emailed to COVIDpolicy@touro.edu.

      7. If you are filing a religious exemption request, please refer to the last two pages. Please fill out the form and notarize as requested.  In addition to your exemption request form, please ask your religion’s spiritual leader to provide you with a letter from them as a testimony of your reason to not vaccinate.  Please email your notarized form and letter to COVIDpolicy@touro.edu.

      8. When submitted, review of your exemption request requires up to 5 business days. You may email COVIDpolicy@touro.edu for updates.
