Your return cannot be completed without:

• Photo ID
• Social Security cards for self, spouse and all dependents

PLEASE MAKE SURE TO BRING:

• All tax forms (W-2, 1099, 1098, etc.)
• 2015 income tax return (prior year’s return)
• Form 1095-A for those insured through the Marketplace
• Bank account information (voided check)
• All receipts for tax-deductible personal expenses
• Day care information English, French, German, Russian, Yiddish and Hebrew speaking tax preparers available. Certain form limitations apply.

To file a joint tax return, both spouses must be present.
